Application Instructions for New Applicants

Thank you for your interest in the federal Special Milk Program (SMP) and/or the Minnesota Kindergarten Milk Program (MKMP).

Important information for ALL Milk applicants:

The federal SMP and the MKMP application must be completed separately from the School Nutrition Program (SNP) application. Applicants will be contacted if additional application information is needed. An e-mail notification will be sent to the sponsoring authority contact when the application is approved. Claims for reimbursement may not be submitted until the approval is complete. Access the SMP/MKMP website for additional information, resources and forms as well as the appeal procedure.

New applicants complete the application process by submitting the following:

SMP/MKMP Program Agreement outlines the responsibilities between the organization and the Minnesota Department of Education (MDE)-Safety, Health and Nutrition (SHN) to participate in SMP/MKMP. Complete all requested information and obtain an original signature of an authorized representative from the organization. Access the Milk Program Agreement.

SMP/MKMP Application provides information about the organization (sponsor) and information on each site location(s) where the milk is served. Access the Milk Program Application.

Cyber-Linked Interactive Child Nutrition System (CLiCS) Site Identification (ID) Request is used to identify the physical location where milk is served. Complete one form for each site where milk will be served. Access the CLiCS Site ID Request Form.

CLiCS User ID/Password Request Form is required to add or delete access to CLiCS. Use this form to request organization staff to have access to submit claims and manage the online application. Access the CLiCS User ID Request Form.

Listserv Subscribe/Unsubscribe Form is required to add or delete organization staff to receive listserv e-mails from communicating specific program information. Submit the name and e-mail address for individuals who should receive information related to SMP/MKMP. Access the Listserv Subscribe/Unsubscribe Form.

Dun & Bradstreet Data Universal Numbering System (DUNS) Number is a unique nine-character number that identifies an organization and is used to track how federal child nutrition program funding is allocated. To verify if the organization has a DUNS number or to register for a DUNS number, visit the Dun & Bradstreet website.

Vendor Number is a specific number issued through the Minnesota Office of Management and Budget (MMB) that will be used for all transactions and contracts. To register with the state for the first time and to access additional vendor information, visit the Minnesota Management and Budget website.

Private non-profit applicants only: Submit a copy of the organization’s IRS 501(c)(3) designation.

School Food Authority applicants:

A School Food Authority (SFA) applicant currently participating in the SNP and planning to use existing SNP sites may complete the milk application online in CLiCS. Current SFA applicants do not need to submit a Milk Program Agreement or obtain a DUNS and vendor number.
