Coordinating Data Collection

Designate one person in each Early Childhood Family Education (ECFE) and/or School Readiness Program to be responsible for coordinating the demographic data collection and submission. This individual will distribute and collect the Early Childhood Family Education and School Readiness Participant Questionnaire forms (ED-2309-11) and tally results on the Minnesota Department of Education Participant Questionnaire Summary Worksheet forms ED-2006-22 (ECFE) and ED-02150-19 (School Readiness) by July 15, 2014.

Completing the ECFE/School Readiness Participant Questionnaire

Each family enrolled in ECFE and/or a School Readiness program during the 2013-2014 school year should complete the “Early Childhood Family Education and School Readiness Participant Questionnaire” form (ED-02309-11). An enrolled family is one in which at least one parent of one child participates in two or more sessions, activities or educational home visits. In order to obtain a high rate of response, please encourage every family participating in the program to complete the survey. Many programs find it easier to have a family member complete this form as part of the regular registration process. Be sure to collect data from all participating families including those enrolled in special programs that may be done in collaboration with other groups (e.g., Head Start, Early Childhood Special Education, or community-based preschool programs) or agencies.

All data is voluntarily provided by program participants and must be kept confidential.
