Form A (Information from the Interpreter): In order to begin the process to request an extension, you must list/contact four (4) individuals to respond to your request by filling out their names on Form A. On Form A, please list names of the mentor with whom the interpreter worked, a parent of a child that the interpreter serves, the special education director where the interpreter is employed and a representative from the Regional Service Center for the Deaf and Hard of Hearing (now renamed Deaf and Hard of Hearing Services). Please also prepare and complete a packet for each individual to review.
On the back of Form A, include a written rationale for the extension and a detailed proposed solution, including timelines. If there is a medical-related issue, please attach medical documents with the doctor’s signature. When the form is completed, please send it to the Minnesota Department of Education: Deaf and Hard of Hearing.
Interpreters are responsible to see that this is done.
Form B (Information from four (4) other sources): Before mailing this form to the four (4) individuals, please fill out the interpreter’s name on part A of Form B. Each individual will send the form back to the Minnesota Department of Education: Deaf and Hard of Hearing. Please also prepare a self-addressed envelope for each individual to mail Form B to the Minnesota Department of Education: Deaf and Hard of Hearing.
Once all of the forms are submitted to the Minnesota Department of Education: Deaf and Hard of Hearing, the committee will determine the length of the extension based upon each individual’s situation.
The review committee will forward their recommendations to the Commissioner, Minnesota Department of Education (MDE). The Commissioner will send the interpreter a letter outlining the extension. Expect this letter by the beginning of August.
